Policyholders use this form to provide a Proof of Loss to their insurer, which is the policyholder’s statement of the amount of money being requested, signed and sworn to by the policyholder, with documentation to support the amount requested.

While this description states that the form must be sworn, NFIP guidelines state that the insured’s electronic signature is sufficient. Remember, when submitting the Proof of loss, the insured must also submit the supporting documentation for the amounts contained in the Proof of Loss.

The current deadline for delivering the fully completed Proof of Loss with all attachments and backups included with the Proof of Loss to the NFIP carrier is 120 days from both Hurricanes Helene and Milton.

Hurricane Helene:  Date of loss was September 26, 2024, so the deadline is Friday, January 24, 2025.

Hurricane Milton:  Date of loss was October 9, 2024, so the deadline is Thursday, February 6, 2025.
